Our Onboarding Process

Since the beginning of our story, we knew that offering personalized services and care to our community members would be the differentiator between us and other companies.  At the heart of who we are, providing joy and heart in every step of the process was non-negotiable because our community members and their families are like family to us.

So read on to get our detailed onboarding process, info on how we work, and next steps for your family.

Searching for the right environment for your loved one to be cared for is an intimidating process filled with many questions:

Since we rebranded our look and site, we knew we wanted our blog to feature answers to your most pressing questions: our onboarding process – for new community members. It can get overwhelming – we know! We are here to make the process as easy as possible for you and to bring you peace of mind and joy to you and your loved ones’ lives.

So read on! You can print out this post and use it as a checklist to get you started with us. Remember, you can contact us directly should you have any questions or concerns.  


STEP 1: Get in Touch

First things first – contact us HERE for a free phone consultation with our intake specialist, Rayetta, to discuss your specific needs and services. We don’t operate by a cookie-cutter approach so it’s vital that we talk about how we can customize support plans and help you and your loved one best. STEP 2: Application + Interview Process

Once you’re ready to move forward with HITH, we head on to the Intake Process. It looks like this:

1 – Application. We’ll send you a link to our application, an access code to our Client Portal System, and an invoice for a $200 Intake Fee. Once the application and fee are complete, our next step is a…

2 – Face to Face Interview. We’ll schedule a meeting to get to know you, your needs, and your loved ones’ needs. We’ll fill out forms (Client Home Safety Assessment and a Release of Information Form) to help us gather your loved one’s history and then we’ll build a plan with our Support Team. Within five days of gathering all this information and forms, we will contact you with a Proposal of Services which could include self-care skills, like teaching healthy eating habits, monitoring blood sugar,  understanding daily hygiene, and taking medications as prescribed by their doctor.

Regardless of the type of services we recommend, our staff will ensure all necessary services are carried out including medical care, physical therapy, occupational therapy, vocational training, education, and mental health services.


STEP 3: Welcome to the Family

Once you review the Proposal of Services, we create a monthly contract customized to the proposal. Payment is made in advance to start services. **Need our services quickly? We can begin providing services within 24-48 hours if needed.

This concludes the onboarding process. Our goal for the onboarding process is to streamline your loved one so he or she can get the help they need quickly and effectively. Above all, we are client-centered and team guided, and we work together (with you, too) to create the best plan of support for our families.
